If you teach a Special Day class in a district that has purchased and N2Y, Unique Learning System (ULS), or Polaris and have to complete 3-year reports, this resource is for you! This Report makes it quick and easy to fill in your students levels and report to parents in the triennial meeting.

 

This is an Academic Evaluation Report Template for Special Ed Teachers who give the updated Unique Learning System (ULS) Benchmarks as part of an Initial or Triennial/reevaluation for Special Education.

 

This resource can be used for writing an informal academic report for students with more extensive needs (formally Moderate/Severe special needs). This can be presented to parents in the IEP meeting. It can be reported on individually or added to a multidisciplinary report with all service providers and team members. It incorporates all the new benchmarks as well as their definition and information on testing formats, versions, and the reading fluency assessments (accuracy and comprehension) with spots for a summary. This resource will be updated as ULS continues to add new benchmarks.

 

This template includes the following information:

1. Description of ULS Benchmarks and testing

2. Observation during testing section

3. Interpretation Descriptions (i.e. Proficient, Instructional etc.)

4. Reading assessment scoring rubric

5. Format/Versions Description

6. Easy to read tables for all available ULS Benchmarks, their description, and places to input student's benchmark interpretation and score

7. Easy to read Table for Observational Profiles and their descriptions

8. Designated spots to insert Observational Data and Polaris Profiles/Rubrics in for Adaptive/functional, Social/Emotional/Behavioral, Cognition, Daily Living, and Community Living skills

9. Description and table for K-12 Student Learning Profile

10. Template summaries for ELA, Math and overall Summary to easily edit.

 

This resource is 25 pages but you will most likely not be giving every single benchmark from ULS, which means your report will be shorter and you have the ability to give as many or as little tests as you choose! Delete what you don't need! The best part is that you can use for many students who have a variety of needs and levels.
