This is an Informal Academic Evaluation Report Template for Special Ed Teachers who need to take classroom data and report it to parents.
This resource can be used for writing an informal academic report for students with more extensive needs (formally Moderate/Severe special needs). This can be presented to parents in the IEP meeting. It can be reported on individually or added to a multidisciplinary report with all service providers and team members. It incorporates 152 academic and functional skills descriptions in the areas of reading and math as well as places to state expressive or receptive formats. This with spots for a summary.
This template includes the following information:

2. Observation during testing section
3. Interpretation Descriptions (i.e. Proficient, Instructional etc.)
4. Reading assessment scoring rubric
5. Format/Versions Description
6. Easy to read tables for all 152 skills, their description, and places to input student's skill interpretation (proficient/instructional/emerging) and percentage score
7. Easy to read independence table for observational skills and a accompanying detailed editable template summary section in the following areas:

This resource is 18 pages but you will most likely not be reporting on every skill that is described in this document, which means your report will be shorter and you have the ability to decide how many skills to report on! Delete what you don't need! The best part is that you can use this for many students who have a variety of needs and levels.
